Description
This
eLearning series provides you with guidance on transitioning individuals with serious
mental illness (SMI) and medical challenges to community living. These brief
modules provide content to support your agency in implementing a program to
successfully transition individuals from institutional living to community
living. This training contains 4 modules and is estimated to take 1.75 hours to complete.
- Module 1 – Introduction to Money Follows the Person
- Module 2 – Implementation of CAT
- Module 3 – Collaboration
- Module 4 – Relocation
Upon successful completion of the modules, you will earn a certificate of completion. This training provides 1.75 Continuing Education Credits for the
following licenses: Psychologist, Professional Counselor, Psychological
Associate, Social Work, Marriage and Family Therapist, and Chemical
Dependency Counselor. Upon successful completion of this online
course, you will earn a certificate of completion and (if applicable)
continuing education credit.
