Training Dates: November 24, December 4, December 10, and December 12

This interactive virtual training will train organizational trainers to equip service coordinators with the knowledge and practical skills needed to:

1. Effectively implement the new STAR+PLUS HCBS form and process.
2. Apply PCT principles and tools to service planning practices to ensure the person’s preferences, values, and goals are reflected throughout the person-centered service planning process.
3. Strengthen collaboration with the person, family members and others who support the person, and interdisciplinary team members to ensure seamless implementation of the new form and process.

Who Should Attend:
Trainers who oversee or conduct training for service coordinators responsible for person-centered service planning (facilitation, documentation, and monitoring) within their organization. Completion of required Person-Centered
Thinking (PCT) training and refreshers is required prior to attendance.
