This virtual training is designed to strengthen the ability of
service coordinators to apply Person-Centered Thinking (PCT) principles,
skills, and tools when completing Form 1701, Support Plan Narrative,
aligning with Federal requirements for the person-centered
service planning process and person-centered service plans, codified
in 42 Code of Federal Regulations Section 441.301(c).
This interactive virtual training will equip service coordinators with the knowledge and practical skills needed to:
- Effectively implement the new STAR+PLUS HCBS form and process.
- Apply PCT principles and tools to service coordination planning
practices to ensure the person’s preferences, values, and goals are
reflected throughout the person-centered service planning process. - Strengthen collaboration with the person, family members and
others who support the person, and interdisciplinary team members to
ensure seamless implementation of the new form and process.
Who Should Attend:
Service coordinators who are responsible for facilitating,
documenting, and monitoring the person-centered service planning
processes, and who have completed the required initial Person-Centered
Thinking (PCT) training and refreshers.
