This module is designed to provide you with an understanding of the relocation process from the perspective of the CAT Therapist when supporting individuals with serious mental illness (SMI) who are transitioning from a nursing facility into the community.
Upon successful completion of the module, you will earn a certificate of completion, but no continuing education credit is offered.
